DUSO Champs program shout outs (optional)

As part of the 2025 Dutchess Ulster Swim Organization Championships (DUSO), we are creating a souvenir program. In addition to celebrating the event, the program will include "Messages from the family/friends" section in our Championship Programs for families to purchase.
The Messages from family/friends section will allow DUSO families/friends to send messages to their swimmer and his/her team.

Please have forms completed NO LATER then July 24th
Messages can be purchased in 4 sizes: Business Card size, ¼ page, ½ page, and full page. The program will be a standard 8.5 by 11 program. Fees for the messages are as follows:
- Business Card = $5
- ¼ page = $ 15
- ½ page = $ 25
- Full Page = $50
